Overview
Chronic coronary syndrome encompasses patients with established coronary artery disease outside the setting of an acute coronary syndrome.
The clinical picture varies from stable exertional symptoms to prior myocardial infarction or previously documented coronary disease.
Risk Factors
Major risk factors include smoking, hypertension, diabetes, dyslipidemia, obesity, age, family history, and chronic kidney disease.
Assessment
Assessment combines symptoms, cardiovascular risk, resting ECG, functional testing or anatomical imaging when appropriate, and assessment of ventricular function.
Management
Long-term treatment focuses on symptom control and prevention of cardiovascular events through lifestyle measures, preventive pharmacotherapy, and revascularization when indicated.
Clinical Pearls
Chronic coronary disease requires two simultaneous strategies: control ischemic symptoms and reduce future cardiovascular risk.
